HEALTH FLEX INCENTIVES FOR 2007
The Conference Board of Pensions has decided that we will offer in 2007 and 2008 the incentive program that HealthFlex is offering to us. For 2007 in order for conferences to receive a rebate on the premiums for our Active PPO plan (generally persons age 65 or under who have not retired from active ministry) our participants have to take the Web MD survey and also have an annual wellness exam (or annual physical) during the calendar year 2007 . We are encouraging persons to take the survey and have their annual exam. If you need help getting started taking the Web MD survey, call the General Board of Pensions at 1-800-851-2201 and chose the option related to “health”. The General Board of Pensions will answer questions and help you get into Web MD. If you have taken the Web MD survey in the past, we need you to take it again during 2007. One important note about the “annual wellness exam” portion of the incentive – your doctor has to submit the claim using the correct “wellness code” in order for the Gen. Bd. of Pensions to count it as a “wellness exam”. We have included the “wellness codes” on the back of this page for your use. Please take it to the doctor when you go for your annual physical. In addition, we have the age appropriate guidelines for an “annual wellness exam” and are glad to mail or fax them to you. Call our office at 1-800-535-4224 and ask for a copy of the guidelines for your age group. These will be added to the Administrative Services section of the South Georgia Conference web site this month so that they can be printed off at your convenience.
The incentive that we receive will be based on our participation in both the “annual wellness exam” and in the Web MD survey. Any rebate that the conference receives will be distributed to the church and the participants in the spring of 2008 after the conference receives it from the General Board of Pensions. For 2007 our thresholds of participation has to be greater than the participation in 2006 so we need as many persons participating as possible.

Wellness Exams Completion Criteria
Providers should use one of the following primary diagnosis codes (ICD9) and CPT procedure codes (CPT4) to ensure completed wellness exams are counted toward the HealthFlex Incentive Program.
